IMPORTANT: To participate, you must register for a time slot by October 16th at 10 am. We will turn in the final head count at this time.

What you will need:
–Your completed vaccination form– must be filled out before arriving for your appointment.
–A copy of your Medicaid card or insurance card

Medicaid covers the flu shot for anyone over the age of 18. ASN’s insurance through Arkansas BlueCross BlueShield will cover the cost of the shot. Most other insurance companies will also cover the cost of flu shots. If you are unsure if your insurance will cover the cost you can call your insurance company or Walgreens at 479-756-1290. Without insurance, the price of the shot is $31.99.
